Many health insurance plans require referrals to specialists. To request a referral, you generally need to visit your primary care provider first.

For an Interpreter, Just Ask!

The Everett Clinic offers interpretive services upon request at no additional charge to our patients. Let the receptionist know if you need an interpreter when you book your appointment. For telephone communication, Washington Relay is a free service provided by the Washington State Office of the Deaf and Hard of Hearing. Dial 711 to connect with a relay operator.

Need to Cancel or Reschedule? Please give us 24 hours’ notice.

Please call the department where your appointment is scheduled at least 24 hours in advance or you may be charged for time scheduled. The Everett Clinic may choose not to see those who repeatedly cancel appointments without notice.

 Full Name (including middle initial)
 Date of Birth
 Current Address and Phone Number
 Physician’s Name (who you would like to schedule the appointment with)
 Purpose of the Appointment
 Current Insurance Information
 Current Employer's Name & Phone

It is important to let the scheduler know about ALL of the concerns you would like addressed so that we can schedule the appropriate amount of time to care for your needs.

Please bring the following with you to your first appointment:

 Completed Adult Medical History Form
 Completed Medical Record Release Form
 Your Insurance Card (and/or coupon)
 Your Photo ID
 Your Copay (if you have one)
 Any Prescription Medication You are Currently Taking (or a complete list)
 Change of Address or Phone Numbers (if you’ve recently moved or made a change)
