What information will the receptionist need to schedule an appointment?
When you call to schedule an appointment please have the following patient information ready:

Full Name, including Middle Initial
Your Current Phone Number
Your Birthdate
Your Physician's Name
Your Insurance Card
The Nature or Reason for Call/Purpose of Appointment

It is very important that you let the scheduler know about all the concerns you would like addressed during your appointment. This helps us make sure an appropriate amount of time is scheduled. COVERAGE REMINDER: If you are scheduling a regular well exam and additional care is provided other than what is normally covered in routine well exams, your insurance may require an additional copay or deny payment and you will be billed for these services. Please check with your insurance provider for more information about your coverage.
What should I bring to my appointments?
Please bring:
  • Your insurance card and/or coupon
  • Your copay (if you have one)
    NOTE: Copays are required to be paid at the time of service otherwise there is a $15 late fee. If you forget your copay, ask the receptionist to provide you with a postage paid envelope to send it to us within 24 hours of your appointment.
  • Any prescription medication you are currently taking (Bring in your medications or write them down and bring a complete up-to-date list)
  • Change of address or phone number(s) if you’ve recently moved or made a change

NOTE: Always arrive 10-15 minutes early for your appointment. If this is your first appointment please arrive 20 minutes early for preparation of your records and to complete paperwork.
What if I can’t make my appointment or I need to reschedule?
Please call your physician's office as soon as you realize you won't be able to make your appointment. We understand situations arise. If you do not notify us within 24 hours of your reserved time, we may be forced to charge you for the appointment scheduled. For repeated insufficient notification, we reserve the right to discontinue service.
What if I’m late for an appointment?
You will be offered options depending on your physician’s practice.  The possible options are: 
1. reschedule your appointment
2. make an appointment later in the day if available
3. make an appointment with another provider if available
Will I always be able to see my own doctor?
In most cases you will see your own doctor. However, there may be occasions when your doctor is out of the office or is unable to see you due to a full schedule. We may then ask if you would be willing to see another physician that day. If, on rare occasion, we are not able to see you for an illness in our office we will refer you to one of our Walk-In Clinics.
How do I get my blood pressure checked?
Main Campus blood pressure checks are provided in the Gunderson Building, Tuesdays from 9 a.m. to noon on a drop-in basis. If you receive care at any of our satellite locations, please call to see how your location handles blood pressure checks.
What about shots/injections?
Shots/injections at Main Campus are done Monday through Friday from 10 a.m. to 11:30 a.m. and 2 p.m. to 4:30 p.m. on a work-in basis. Please allow time as these injections are given between other patient care tasks. If you receive care at any of our satellite locations, please call to see how your location handles injections.
If I call with a question for the nurse or doctor, when can I expect a call back?
Our physicians and nurses are with patients throughout the day, so it is usually not possible for them to immediately answer individual phone calls unless it is an emergency. To help us get back to you as quickly as we can, please remember to give your full name including middle initial, the nature or reason for the call, your birthday, current phone number and current insurance carrier.
  • If you call between 8 and 10 a.m. we try to return your call between 12 and 2 p.m.
  • If you call between 11 a.m. and 3 p.m. we try to call you by 6:00 p.m.
  • If you call after 4 p.m., we may need to call you back the next day depending on the nature and urgency of the call.
What about after hours care?
If you are calling after 5 p.m., the answering service will receive your call and can page the on-call physician. They will take your phone number and the nature of your call and will have the on-call doctor call you back. When the doctor calls you back he/she will instruct you as to whether yo need to go to an emergency room or a walk-in clinic facility for immediate care or if the problem can wait for an appointment the next day. You may also want to have your pharmacy information handy in case he/she calls in a prescription for you.
