The Geriatric Extended Care team provides day-to-day medical management for Everett Clinic patients in nursing homes in Everett, Marysville, and Snohomish. They provide after-hospital care in subacute and skilled care units, as well as long-term care for those unable to return home.
Our team consists of a physician and nurse practitioners that provide on-site medical care in collaboration with each patient’s primary care physician. The goal of care is to allow patients to achieve and maintain their highest level of function.
