How the PCT works for you

Together, the team offers complete care to people with more complex medical problems.

The team includes a variety of providers you’ll often see during one visit. The PCT works hard to offer best-in-class care while possibly lowering your overall cost of care.

If you qualify to be part of the PCT, your primary care doctor will let you know.

Members of the PCT include a physician team with a doctor and an advanced practice clinician, such as an advanced practice nurse practitioner (ARNP) or a certified physician assistant (PA-C).

PCT members may also include:

Clinical pharmacists (medicine experts)
Complex care navigators
Medical assistants
Nurse care managers
Palliative care support
Social workers

Care with the PCT includes benefits, such as:

Convenient virtual care options by video or phone
Dedicated social workers
Extra support and attention from your care team
Longer appointment times so you can see several providers in one visit
Lower overall care expenses, in most cases

When you see the PCT, you should have fewer visits to the ER and clinic. Fewer visits may mean fewer bills.

The PCT is at the Everett Campus and Marysville, so you may need to change where you get care. And remember, a PCT member may also visit you at home, online or on the phone.
