Please join us for a facilitated group conversation about advance care planning at the Stanwood Senior Center in the Social Room (7430 276th St NW, Stanwood 98292). Family members are welcome and encouraged to come.
This workshop covers advance directives, power of attorney and POLST forms, and other end of life related issues, including specific instructions on how to complete these forms. There is time for questions and answers with our experienced team of providers.
