The geriatric extended care team cares for our patients at local skilled nursing facilities in Everett, Marysville and Snohomish. They work to keep Everett Clinic doctors and staff up-to-date on their patients. They also offer:
- After-hospital care in sub-acute and skilled care units (These are places you may go to when you're well enough to leave the hospital, but still need special care.)
- Long-term care if you or your loved one is unable to return home
Your health care team
Your team is made up of a doctor and nurse practitioners that offer on-site medical care. They work together with your primary care doctor. Their goal is to help you or your family member be as healthy as possible.
